Your First Visit
During your first visit, I will conduct a thorough examination of your mouth, teeth and gums. The exam may include necessary x-rays to accurately determine the condition of your oral health. Your teeth may also be cleaned and polished at your first visit depending on what your particular oral condition requires. I will listen carefully to your dental concerns and answer all questions thoroughly. Following a careful diagnosis, we will discuss a treatment plan that is suitable for you.
Your comfort, appearance and long-range oral health needs will be my primary focus. When treatment is indicated, I will strive to restore optimum dental health in as few well-planned appointments as necessary.
I recognize the value of your time. Except in emergency situations, you can expect me to be on time for you. If you are unable to keep your appointment, I ask that you notify us 48 hours in advance.
Under the “New Patient Forms“ tab, you will find patient registration forms and a health questionnaire. Please print and complete them and bring them with you to your first visit.
If you have dental insurance, please bring your insurance card as well. We will gladly submit your claim for you. Your anticipated portion of fees is due at the time of service. If you do not have dental insurance, payment for the first visit is appreciated at the time of service. Payment plans are available for extensive treatment plans. For your convenience, we accept Visa, Master Card, Discover and American Express.
Please call the office or email me if you have any questions. I look forward to a relaxed and pleasant visit with you!
